Dr. Ann Bellowes catches her husband and fellow therapist having sex with their couples counselor. Ann's colleagues in the office help her to deal with it as they deal with their own issues.
Ann asks Phil to come to dinner and help her break the news of their separation gently to her visiting mother. Her mother is hurt and upset that Ann didn't tell her sooner. Ann helps a transgendered girl to deal with being a bridesmaid in her sister's wedding. Meanwhile, Taj helps a suicidal boy.
Ann discourages her patient from contacting the son she gave up at birth, but the patient does it anyway. Barry's criminal father sends him another client and Ann is upset when the client arranges to film the reunion of Ann's patient and the patient's son. Meanwhile, James' ex-wife learns that she doesn't have a fatal illness after all.
Ann and Phil fight over the division of their assets. Upset, she gets into a crash on her way to work and meets Cordelia's ex-husband who she then goes on a date with. Meanwhile, at work Cordelia struggles with a new patient who is falling for her and Ann deals with a couple who keep showing up for counseling with injuries.
Taj has the colleagues over for dinner. While there, Ann notices that Taj's daughter may have an eating disorder. She tries to talk to Taj about it, but when that fails she talks to his wife. Barry struggles with a custody dispute involving two lesbian parents. Cordelia tries to tell Taj that his wife knows about their affair, but he doesn't believe it until his wife casually drops it in their conversation about their daughter's eating disorder.
When an old friend needs surgery for a brain tumor, he asks Ann to take custody of his daughter, Hazel, should anything happen to him. Meanwhile, Cordelia believes her patient is stalking her, but later learns that he's trying to blackmail her and Barry deals with his criminal father.
Ann's sister drops in on Ann at work with a crisis--she's having an affair and wants to leave her husband, but she wants Ann's advice and approval. When Phil comes by with some old pictures of Ann and him, they have a brief reconciliation. Taj and his family go to therapy to deal with his daughter's possible eating disorder. Meanwhile, James works with a boy who has stopped speaking after his brother's suicide.
James' father is kicked out of his retirement home and James asks his friends to help him keep an eye on his father. Meanwhile, Ann deals with personal issues as well.
